1. This Proposal supersedes any prior contractual agreement with Sustainable Pest Systems, Inc concerning the property and/or properties listed therein.

2. Services required other than indicated pests and/or exterior control procedures and/or change in frequency of service, would involve additional charges unless otherwise indicated.

3. This proposal excludes stores within apartment houses and office buildings unless otherwise specified.

4. No additional treatments will be made for tenants or lessees unless Sustainable Pest Systems, Inc is so authorized by owner.

5. This proposal will be considered an agreement for one year when signed by the client.

6. This proposal can be amended by Sustainable Pest Systems upon thirty (30) days' written to the owner and acceptance thereof from owner's receipt of the amendment within ten (10) days.

7. This proposal will continue until terminated after the contract term by either party upon thirty (30) days' written notice.

8. It is understood that pests are introduced on an ongoing basis (i.e., on supplies, by employees, from surrounding establishments and the exterior). Effective rodent and insect management is contingent upon good housekeeping, effective pest-proofing, as well as good sanitation inside and around the treated structures. If a zero pest population is to be maintained, you must conform to any recommendations submitted by Sustainable Pest Systems, Inc. Failure to do so may result in voiding any warranties.

9. It is expressly understood that Sustainable Pest Systems, Inc is not responsible for any damage heretofore or hereafter caused by insects or rodents whether of the same or different character from rodents or insects previously found in the premises and that no warranty or representation to the contrary shall be of any force and effect unless signed by the President of Sustainable Pest Systems, Inc nor shall Sustainable Pest Systems, Inc be liable in any way for any consequential damages by reason of insects or rodents, including without limitation loss of use or loss of profits.

10. It will be the obligation of Sustainable Pest Systems, Inc to perform treatment procedures and select chemicals in conformity with Federal, State and Local regulations and agencies governing said practices.
